Retarded Ejaculation

This sexual disorder is suffered by a minority. It is the antithesis of premature ejaculation. Retarded ejaculation refers to the extreme difficulty to ejaculate despite sexual stimulation. This can take place in different degrees. Suffering is slower than in the case of males with premature ejaculation, but it can be experienced very intensely. This, obviously, aggravates this problem more and more. In fact, it can derive in depressive symptoms which, caught in time, can be easily resolved.

For many men facing this problem, the suffering has to do with how much effort they need to ejaculate and multiply their doubts about whether they will get it next time. For other men, what happens is that they cannot ejaculate during sexual intercourse. They can even ejaculate by themselves, but not in the presence of their partner.

In more severe cases, the man has never ejaculated except in dreams. It can also manifest itself among gay males and it may include the impossibility to ejaculate with his partner, regardless of the sexual interaction.

The distinction between organic and psychological causes of retarded ejaculation is very important. If the delay is situational, that is, if it does not happen always and if it is fine during masturbation, medical causes can be discarded. It means it obeys to psychological reasons.

Among the organic causes we have the following:

-      Side effects of some pharmaceutical products that can become out of the treatment for the inverse problem (premature ejaculation).

-      Neurological lesions: medulla lesions (tumors, traumatisms, multiple sclerosis…), interventions such as sympathectomy, neuropathies as a consequence of alcoholism, diabetes, uremia, etc.

For the psychological causes, often the ejaculatory delay usually is primary. That is, the most frequent profile is that of a young man with obsessive personality traits, that have experienced the delay or impossibility to ejaculate forever and in any situation.

Among the psychological reasons that produce retarded ejaculation we find behavioural mechanisms such as an obsessive self-consciousness or the exaggerated attention to his sexual partner; factors related to the ambivalence with his partner and, perhaps, with women in general. Or there can be deeper psychological causes, such as fear to intimacy, fear to make commitments, or conflict with his sexual orientation (difficulty accepting his homosexuality).

Recent studies have shown that a very common trait among men who suffer from retarded ejaculation is a certain emotional blockage.

Curiously enough, treatment for this dysfunction is complicated, among some reasons, by the patient’s difficulties to make a commitment to adhere to therapy to the end. However, therapeutic work is usually very interesting and enriching both for the patient and the therapist. Results can easily compensate the efforts involved in resolving it.

The length of the treatment will depend on the severity of the problem. When the orgasmic inhibition is not severe, treatment is more or less simple. However, these types of patients often decide to ask for help when the problem has become serious.

Distinctive Features of Ejaculation Disorder Connected With Psychological Problems

 - beginning of sexual life
 - irregular sex life during which are alternated periods of high activity with long-term abstinence
 - duration of sexual intercourse may be different
 - with different partners intercourse occurs differently and lasts different period of time
 - frequently after first intercourse it is impossible to continue intimate interaction due to lack of normal erection
 - problems with ejaculation often do intercept with erectile dysfunction
 - it is possible to prolong sexual intercourse from time to time mentally switching to abstractive themes, often unpleasant. Sometimes this way to abstract from reality leads to erection disappearing.
 - alcoholic drinks help to relax but they delay ejaculation.

Men's Health

It is considered that men's health is about andrology and urology, but in fact the issue is much broader.

Testosterone is a hormone which makes a man. Testosterone in male body works on male genitalia, prostate gland, seminal vesicles, epididymis, penis; activates metabolism of fats, proteins, cholesterol, microelements; normalizes spermatogenesis, stimulates sexual manifestations and other processes.

Testosterone mainly has its effects on formation and activation of male libido, sexual arousal and also intensity of orgasm. With age (starting from 30 years old) in male body testosterone level decreases. At this time, do start problems with health, muscles stop growing, and instead, grows belly and do appear wrinkles.

Testosterone deficiency has negative effects on almost all organs and systems in male body. Testosterone is called “hormone of happiness” and its deficiency affects not only physical and emotional condition of a male – he experiences bad mood, depressions and unmotivated pessimism. Also, gradually decreases the ability for intellectual thinking along with working ability and physical endurance, and besides this, sexual activity lowers.

Most men note symptoms of sexual disorder such as erectile dysfunction. Most often men think it is sign of male weakness or the result of aging. However, in fact, erectile dysfunction is the result of vessel walls disorder in the whole male body and not only in penis. Major changes occur in next systems: cardiovascular, urogenital, central nervous system, musculoskeletal system, and skin. Coronary disease signs show that it is primary stage of circulation disorder not only in pelvic organs but also in whole cardiovascular system and other organs. Unfortunately, usually patients seek doctor's advice quite late.

What symptoms should put you on the alert?

  • excessive sweatiness
  • frequent palpitation
  • easy fatigability
  • disturbance of memory
  • production loss
  • sleeping disorder
  • hair loss
  • wrinkles
  • sexual activity lowering
It is very important to stop timely testosterone level drop in order to slow down developing damage of vessels. You should see doctors (andrologist, cardiologist) regularly for preventive examinations.

Short Coitus

Short coitus is a condition when sexual intercourse lasts long enough to be called premature ejaculation but its duration not satisfyes both partners. As a rule, short coitus lasts from 2-3 to 10-15 minutes.

Short coitus is not a disease but this condition can be corrected.  Causes of short coitus are not known yet. It may have the same causes as premature ejaculation but may not be as frank. Frequently causes are not connected with intercouse.

Not in all males this problem can be solved. When are found causes of this condition (hypersensitive glans, chronic vesiculitis) it is necessary to get rid of them. If causes are not found, you should follow common recommendations to increase duration of sexual intercourse.

How to increase duration of sexual intercourse?
1) Try to use condom. You may use 2-3 condoms in the meantime.
2) Try to think about abstract things during sex. Sometimes help thoughts about nasty things.
3) Make yoga exercises. It is necessary to learn to delay and start urination easily. Generally speaking, the connection between sphincteric mechanism of bladder and ejaculation is hardly known but this exercise helps in some cases.
4) You may try to masturbate before sexual intercourse, especially if you had last ejaculation long ago.
5) Most effective recommendation – make circumcision. It helps any man to prolong sexual intercourse by 2- 3 times.

What should you not do to delay ejaculation?
Do not squeeze urethra with your hands. Sperm remains in the back part of urethra, part of it goes again into seminal vesicles and other part of it goes into bladder. Permanent squeezing results in chronic conditions in prostate gland which may lead to chronic prostatitis. Also it affects work of prostate gland sphincteric mechanism which leads to retrograde ejaculation.

If you see that even after following these recommendations the problem is not solved,  you must go to the urologist.

What Is Erectile Dysfunction?

Erectile dysfunction (impotence) is a condition when penis gets erected incompletely. Term “erectile dysfunction” is used to describe full erection inability, erection instability or ability to short erections.

In other words, erectile dysfunction is a persistent inability to get erection, necessary for a normal intercourse. Erection is a difficult process in which are involved emotions, circulatory system and three independent parts of nervous system.

How much dangerous is erectile dysfunction?
Erectile dysfunction usually signalizes about serious disorders in the body.

Erectile dysfunction causes
There are many causes of erectile dysfunction, which can be divided into six groups;
1. Psychological. As specialists say, in 10-20% of cases of erectile dysfunction play role such psychological factors as stress, anxiety, feeling of guilt, depression, low self-esteem and failure in the bed.
2. Taking medications. Smoking, alcohol and taking drugs leads to hypoactive desire. Taking different medications such as antidepressants, ataraxics, allergy medicines, blood pressure lowering medications may negatively influence on erection.. Ask your doctor if there are such medications among medications you take.
3. Vascular diseases, blood circulation disorder. In atherosclerosis, the artery walls become less elastic. This can lead not just to heart attacks and blood-stroke, but also to problems with blood supply in penis.
4. Neurological diseases. On human nervous system can have negative influence diabetes, traumas, spinal cord diseases, prostate gland operations.
5. Endocrine problems. The level of testosterone can be lowered because of endocrine disorders and other diseases, for example, because of liver cirrhosis.
6. Penis traumas or penis diseases. Erectile dysfunction can be caused by Peyronie's disease or priapism. In the first case, for some unknown reason, occurs proliferation of tissues of penis and as a result penis deformation. Priapism is a long erection, little by little getting down sexual function. One more reason for priapism development are injections increasing erection. In case of appearing of painful erection immediately go to the doctor.

Diagnostics
Feeling of displeasure with erection is a reason to undergo medical examination which will help to reveal possible causes of erectile dysfunction. Many men have erection during sleep. If night erections do exist for a long time then emotional causes are excluded.

Why It Is Difficult To Diagnose Sexual Disorders?

There is no exact definition for normal sexual life. Everybody has its own sexual life that is determined by personal preferences. Frequency varies from several times a week to several times a year.

Sex not necessarily means sexual intercourse. It is not necessarily felling the onset of orgasm in each partner. In fact, any person can have periods of sex cool down. There is no common sex standard, so sometimes it is difficult to say if the couple has sexual problems or not.

There are particular signs which help you to find out if you have sexual problems:

  • the problem occurs permanently or it repeats quite often at length
  • the problems causes stress – it makes man feel upset and greatly worries him
  • the problem results in difficulties in the relationship and breaks down chemistry with sexual partner
Two last signs are most important. In many people  libido change and lowering of sexual function do not cause stress and do not influence on relationship with the partner. In this case there is no sexual problem. However, when a man suffers greatly from these changes, then we can say that the sexual problem exists.

Another difficulty is that sexual disorder is caused not just by one factor, it may be caused by the complex of physiological and psychological factors. Normal sexual function means passing through all sex stages:
1. Excitement
2. Blood flush into genitalia (erection in men, lubricant exudation and genitalia increase in women)
3. Orgasm
4. Feeling of satisfaction and satiation

Breakdown can occur on any stage of this cycle and it can be caused by a number of factors. Finally it results in sexual disorders.

The role of bad habits and diseases
Sexual disorders can be caused by the next factors:
  • diabetes
  • cardiovascular diseases
  • neurological diseases (insult, brain injury and spinal cord trauma, multiple sclerosis)
  • trauma or operation on pelvic organs
  • side effects of drugs
  • chronic diseases (kidney failure, hepatic failure)
  • endocrine profile disorders
  • drinking and taking drugs
  • smoking
  • aging changes
Psychological factors:

  • stress on the work
  • anxiety about problems in the family or with friends
  • mental disorders (depression, anxiety)
  • past unsuccessful sex
Associated diseases and disorders make people worry about their sexual function and namely this problem is added to the existing problem. In case of appearing of serious sexual problems first you should find out if they are connected with taking of some medicines, hormonal disorders, mental diseases (depression, anxiety) or mental trauma.

If it was detected one of these problems then it may be removed by the doctor. Otherwise the relationship in the couple must be analyzed. Sexual disorder may appear just in a certain situation. It means that problem exists just in one partner in a certain situation. In this case both partners should consult psychologist.
